Physical Form

NEW this year is the CHSAA physical form (Spanish) to be completed in order to participate in high school sports. It is preferred for the 2024-2025 school year. It will be required for the 2025-2026 school year. NOTE: The student and a parent/guardian must sign and date the form after the physician completes it in order for it to be a valid physical.
